Health insurance questionnaire on family insurance
Last updated: October 2026
Your children or your spouse are insured for free through you. Now and then the health insurance fund (Krankenkasse) checks whether this still fits and sends a questionnaire. You should send it back in time.

Überprüfung der Familienversicherung
Sehr geehrte Frau Muster,
November 2026 an uns zurück.

What does the letter mean?
In statutory health insurance, children and spouses or civil partners can be insured for free along with you. This is called family insurance (Familienversicherung). There are conditions, for example their own income must not go over a limit. This limit is adjusted every year.
With the questionnaire, the health insurance fund checks whether the conditions are still met. It asks for example about income, a mini job, their own insurance or, for older children, about school or university.
If you do not reply, the family insurance can end. Then the person must get insured on their own and pay their own contributions.
Deadlines
- The deadline for the questionnaire is in the letter, in the sample 30 November 2026. The deadline in your own letter is what counts.
- If you are still missing proof, send the questionnaire in time anyway and write that the proof will follow. Or ask for more time before the deadline ends.
What you can do
- Fill in the questionnaire completely for each insured family member.
- Enclose proof, for example payslips, a school certificate (Schulbescheinigung) or a certificate of enrolment (Immatrikulationsbescheinigung).
- Write your insurance number (Versichertennummer) on every page.
- Send everything by post or through the online mailbox of the health insurance fund and keep a copy.
- If something changes, for example your child starts vocational training or your partner earns more, tell the health insurance fund on your own.
- If you have questions, call your health insurance fund. The advice there is free.
This guide does not replace legal advice. Every letter is different: deadlines and amounts apply as they are written in your letter. If you have questions, an advice centre can help.
